1. Scope of Services

We provide mobile phlebotomy and specimen collection services for patients, clinics, laboratories, and healthcare providers.
We are not a diagnostic laboratory, not a medical practice, and do not interpret, diagnose, or treat any medical condition.
All testing and results are handled by certified laboratories chosen by the client or provider.

2. Appointments

• Services are available by appointment only.

• Appointment times are approximate and may vary due to location or patient volume.

• Additional fees may apply for after-hours, weekend, holiday, or rural-area visits.

• We reserve the right to reschedule or cancel visits due to weather, safety, or operational factors.

3. Payment Terms

• Payment is due at the time of booking or service unless a written agreement states otherwise.

• Accepted payment methods include secure digital or card transactions.

• Once a phlebotomist is dispatched, the service fee and travel charge become non-refundable.

• Refunds apply only if a collection cannot be completed due to a company error.

4. Service Limitations

You understand and agree that:

• We provide collection only — we do not analyze, interpret, or alter test results.

• Specimen handling follows laboratory protocols, but the final processing and results are under the laboratory’s responsibility.

• We are not liable for laboratory delays, rejection of samples, or errors caused by third-party entities.

5. Client Responsibilities

Clients must:

• Provide accurate personal and medical information

• Present a valid laboratory order (if required)

• Follow pre-collection instructions (e.g., fasting, hydration, medications)

• Maintain a safe and appropriate environment for the phlebotomist during home visits

Failure to comply with preparation instructions may compromise results or lead to service denial.

6. Safety & Conduct

We reserve the right to refuse or discontinue service if:

• The location is unsafe or unsanitary

• The client displays aggressive, abusive, or threatening behavior

• Required information or consent is not provided

In such cases, any dispatch or travel fee remains non-refundable.

7. Liability Disclaimer

By using our services, you acknowledge and agree that Labs & Health of Miami LLC, its owners, employees, and affiliates:

• Are not responsible for injuries, bruising, or complications resulting from blood draws or specimen collection

• Are not liable for laboratory testing errors, result delays, or courier handling

• Are not liable for incidental, indirect, or consequential damages of any kind

Our maximum liability shall not exceed the amount paid for the specific service rendered.

8. Medical Disclaimer

All services provided are non-diagnostic and for specimen collection only.
We do not provide medical evaluation, diagnosis, advice, or treatment.
Any medical questions must be directed to your licensed healthcare provider.

9. Privacy & Data Protection

All patient data is handled in accordance with our Privacy Policy and HIPAA standards.
By booking a service, you consent to secure electronic communication (phone, text, or email) for scheduling and follow-up purposes.

10. Indemnification

You agree to indemnify and hold harmless Labs & Health of Miami LLC and its affiliates from any claims, damages, or expenses arising from:

• Misuse of our services or website

• Inaccurate information provided by you

• Failure to comply with these Terms

11. Governing Law

These Terms are governed by the laws of the State of Florida, without regard to conflict-of-law principles.
Any disputes shall be resolved in the courts of Miami-Dade County, Florida.

12. Updates to Terms

We may update these Terms periodically.
Revised versions will be posted on our website with the new “Last Updated” date.
Continued use of our services indicates acceptance of the updated Terms.
