Our Commitment to Your Privacy

Labs and Health of Miami LLC ("Labs & Health") is committed to protecting the privacy and security of your protected health information ("PHI"). We are required by the Health Insurance Portability and Accountability Act (HIPAA) to maintain the privacy of PHI and to provide you with this Notice of our legal duties and privacy practices.

What Information This Notice Covers

This Notice applies to information that identifies you and relates to your past, present, or future health condition — including specimen collection records, appointment details, and any health-related notes you provide. Because Labs & Health is a mobile phlebotomy service and does not perform laboratory testing, diagnosis, or treatment, the PHI we handle is limited to specimen collection logistics.

How We May Use and Disclose Your PHI

We may use or disclose your PHI for the following purposes without your authorization:

• Treatment: to coordinate specimen collection and deliver samples to the testing laboratory you or your provider chose.
• Payment: to process payment for our services through our payment processor.
• Healthcare operations: to schedule appointments, confirm service details, and respond to your inquiries.
• Business associates: with service providers (email, hosting, database) who have signed a Business Associate Agreement (BAA) with us and are bound to protect your PHI.
• As required by law: when disclosure is required by federal, state, or local law.

Any other use or disclosure requires your written authorization, which you may revoke at any time in writing.

Your Rights Regarding Your PHI

• Right to access: you may request a copy of your PHI that we hold.
• Right to amend: you may request a correction if you believe information we hold is inaccurate.
• Right to an accounting of disclosures: you may request a list of disclosures we have made of your PHI.
• Right to request restrictions: you may request limits on how we use or disclose your PHI.
• Right to confidential communications: you may request that we contact you a certain way or at a certain location.
• Right to a paper copy: you may request a paper copy of this Notice at any time.
• Right to file a complaint: you may file a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights without fear of retaliation.

Our Responsibilities

• We are required by law to maintain the privacy and security of your PHI.
• We will notify you promptly if a breach occurs that may have compromised the privacy or security of your PHI.
• We must follow the duties and privacy practices described in this Notice and provide you with a copy.
• We will not use or share your information other than as described here unless you authorize us in writing.

Changes to This Notice

We may change this Notice, and the changes will apply to all PHI we hold. The new Notice will be available at labsandhealth.com/hipaa-notice and upon request.
